Peptide Therapy in Musculoskeletal Medicine

Injectable and regenerative peptide therapies in orthopaedics and sports medicine — what the evidence shows for tendon, cartilage and bone healing, and where the hype outpaces the data.

What it is

Peptide therapy uses tiny protein fragments to help your body heal itself. You might consider this if you have joint or soft tissue injuries. These peptides are designed to interact with your cells to encourage repair.

The science is still developing. There is currently not enough evidence to support their routine use in orthopaedic and sports medicine. While some animal studies show promise, we do not have randomised controlled trials in humans for many of these treatments. This means we cannot yet confirm how effective or safe they are for everyone.

Some peptides may help with specific healing processes. For example, certain types might support tendon-to-bone healing or reduce inflammation in animal models. Others may help with cartilage repair or reduce pain and stiffness in soft tissues quickly. However, the results vary. Some treatments show symptom improvement, while others do not lead to functional recovery in animals.

We do not recommend using these therapies for sports performance or recovery. The evidence is lacking to determine their overall effectiveness or safety profile for athletes. Your doctor will discuss whether any peptide-based option is appropriate for your specific condition, based on the current medical understanding.

Does it work?

The short answer is that the evidence for injectable peptide therapies in humans is currently very limited. Most of the research you will find comes from laboratory studies using animals, such as rats and horses. While these studies show some promising biological effects, they do not prove that these treatments work for people. There are no high-quality clinical trials testing whether these injections improve outcomes for human patients.

For example, some animal studies suggest that certain peptides might help tendons heal stronger or reduce inflammation. However, we cannot yet say if these biological changes translate into less pain or better function for you. In one animal study, a specific peptide hydrogel preserved nerve fibres but did not improve movement. This highlights the gap between what happens in a lab and what you might experience in real life.

There is one exception where topical treatments have shown some benefit. A lotion containing hyaluronic acid and peptides was found to be safe and effective for reducing pain and stiffness in soft tissues. In this case, patients noticed relief within just three hours of the first application, with continued improvement over three days. This is a specific, topical use case and does not apply to injectable therapies for joints or tendons.

Regarding BPC-157, a popular peptide in sports circles, the science is lacking. There are no randomised controlled trials in humans to support its use. Experts do not recommend it for sports performance or recovery because its safety and effectiveness remain unproven.

Your doctor will be honest with you about what the data shows. We rely on treatments that have been tested in people, not just in labs. If a therapy has not been proven to work in clinical trials, we cannot recommend it as a standard part of your care. It is important to manage your expectations and understand that many peptide therapies are still experimental.

What are the risks?

You should know that the current evidence for injectable peptide therapies in orthopaedics and sports medicine is limited. There are no randomised controlled trials in humans for BPC-157, and the science cannot yet confirm its overall safety or effectiveness for sports enhancement. Because of this lack of data, we do not recommend using BPC-157 for performance or recovery.

For topical treatments like EGYFIL, the main risks are local. You may notice pain and stiffness in your soft tissues. This treatment is generally safe, but you might feel discomfort during the three-day treatment period. Some patients report that symptoms improve within three hours of the first application, while others may experience ongoing irritation.

When peptides are injected into tissues, such as for tendon or cartilage repair, the risks are tied to the healing process itself. In animal studies, some peptide applications caused minimal inflammation. However, not all treatments lead to functional recovery. For example, one hydrogel preserved nerve fibres but did not restore movement in animals. You might experience local swelling or pain at the injection site as your body reacts to the material.

There are no guarantees about how your body will respond. The improvement in symptoms you see with some treatments may come from different causes than others. We monitor your progress closely to ensure any local reactions are managed appropriately. If you have concerns about specific side effects, please discuss them with your doctor before proceeding.

Is it right for you?

Peptide therapies are still largely experimental in human orthopaedics. There are no randomised controlled trials for BPC-157 in people. The science cannot yet confirm its safety or effectiveness for sports recovery. We do not recommend BPC-157 for performance enhancement.

Some specific peptides show promise in animal studies. For example, certain hydrogels help tendon healing in rats and cartilage repair in horses. EGYFIL has been shown to reduce pain and stiffness within three hours of application. However, these findings come from laboratory or animal models, not human clinical trials.

You are likely to benefit more from established treatments. Standard options like microfracture or self-assembling peptide hydrogels have shown symptom improvement in equine models. These approaches are currently the more reliable path for structural repair.

Peptide injections are not yet a proven alternative to these standard procedures. Your doctor will discuss whether your specific condition matches any ongoing clinical research. This is a shared decision based on the current evidence.

If you are seeking a proven treatment for tendon or cartilage damage, conventional methods remain the standard of care. Peptide therapies may offer future options, but they are not ready for routine clinical use today.

The bottom line

Injectable peptide therapies currently lack strong evidence to support their use in human orthopaedic care. While some topical treatments like EGYFIL may reduce pain and stiffness within hours, most findings come from animal studies rather than human trials. Your doctor will not recommend unproven peptides for sports recovery or performance enhancement. You should expect that these options are not yet standard practice. We advise discussing established treatments with proven safety and effectiveness instead.
